In my limited experience it sounds like it needs more FOC, you have poor broadhead alignment, inconsistent arrow speeds or not enough fletching offset. You can make your own broadhead alignment checking tool with four marbles and a board as per Awshucks. You can increase FOC by shortening your arrow...
